None Physical Requirements Sitting; driving Shift Monday Friday 8 : 00 a.m. 5 : 00 pm. (40 hours / week). Job Summary Skilled professional needed to join the University of Kentucky (UK) Targeted Assessment Program (TAP).
We are looking for a team member with strong clinical and case management skills. Candidates should be passionate about helping others and advocating for those in need.
The TAP Specialist will be a UK employee co-housed at the Department for Community Based Services (DCBS) office in Hazard, KY.
Duties include : conducting participant assessments in the areas of substance use, mental health, intimate partner violence victimization, learning problems, and basic needs;
connecting program participants with services and resources; providing follow-up to support improved outcomes; and collaborating with community partners.

TAP is sponsored by the KY Cabinet for Health and Family Services through a contract with the University and is supported with Temporary Assistance to Needy Families (TANF) funds.
Skills / Knowledge / Abilities Master’s degree plus 3 years of related experience in social work, counseling, case management, and other supportive services.
