Vantage Healthcare & Rehab at Hampden is now hiring a Physical Therapist (PT) to provide rehabilitation services to our patients and residents. This facility is seeking a caring, warm-hearted Physical Therapist who is currently licensed in Massachusetts or is a graduate from an approved Physical Therapist program and is actively working toward obtaining licensure. Responsibilities:
- Develop and implement physical therapy programs that focus on rehabilitation per state practice act
- Work directly with patients to achieve maximum physical recovery
- Evaluate effects of therapy treatment and communicate patients' progress
- Record and document patient care services accurately and concisely
- Collaborate with other team personnel to achieve well-rounded care
- Graduate of an accredited Physical Therapy Program.
- Current MA State PT License.
- Experience in SNF settings is preferred, but not required.
- Exceptional customer service and patient care experience.
- Ability to work in a fast-paced environment while maintaining a positive attitude.
