Summary
- Department: Stepdown Intermediate Care
- Shift timing: 18:45 - 07:15 Hours Monday to Friday (3 Days/12 Hours)
- Per Week: 36 Hours.
- This department is a 21 bed unit, serving Med/Surg/Telemetry adult patients. Nurse to patient ratio is 1:5-6 on days and 1:6-7 on nights. Care for patients with general med/surg floor needs. The unit is also supported by PCT's and Unit Secretary. Additional Support Mon-Thur includes a Clinical Supervisor on days and nights. Utilization of virtual sitter monitoring is available as well as monitor techs to monitor those on telemetry.
Roles & Responsibilities
- Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Note and carry out physician and nursing orders.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
