Registered Dietitian

Via Health Services needs a Registered Dietitian to assist our residents in their nutritional care and wellbeing.

Our ideal candidate is a self-motivated with a genuine concern for our residents’ well-being and has a positive attitude. The Registered Dietitian coordinates nutritional care of patients by completing nutritional assessments, developing, and implementing care plans and documenting dietary information about the residents. Provides guidance and collaborates with the Certified Dietary Manager (CDM), in the provision of food and nutritional services.

We are looking for a caring and compassionate person who can provide a strong emphasis on rehabilitation and dietary services to become a member of our team. The qualified candidate will be responsible for the delivery of patient care through the long-term and skilled care process of assessment, planning, implementation, and evaluation based on established patient care delivery for the achievement of desired patient outcome. We emphasis teamwork and quality care and provide a strong sense of job satisfaction.

Via Health Services has two facilities located on the south side of Des Moines and in Carlisle serving the long-term and skilled nursing needs for the greater Des Moines area. The Carlisle and Fleur Heights Centers for Wellness and Rehab both support 80 plus beds. In addition, Carlisle’s campus has additional homes for Assisted Living and Independent living.

Job Description:

The Registered Dietitian’s primary responsibility is completing nutrition assessments for all residents and incorporating nutrition interventions into plan of care to maintain and/or restore residents’ to desired nutritional state.

Duties:

  • Completes Medical Nutrition Therapy Assessment
  • Participates in the facility Quality Improvement/Quality Assurance process.
  • Attends mandatory in-services and participates in training programs to enhance skills.
  • Maintains on-site regular attendance as scheduled.
  • Performs all other tasks or duties as assigned.
  • Develop and implement care plans for residents.
  • Development of nutritional assessments
  • Assess residents for nutritional status and provide interventions and treatments
  • ​Collaborate with Physicians, nursing staff and ancillary services such as Physical Therapy

Qualifications:

  • Graduate of an accredited bachelor’s dietitian program
  • Registration and Licensure required.
  • 2 years’ experience working as a Registered Dietitian; preferably in Long Term Care
  • Must be adept at problem-solving, including being able to identify issues and resolve problems in a timely manner
  • Must possess strong interpersonal skills.
  • Must be able to communicate clearly, both written and orally, as to communicate with employees, and residents as needed.
  • Must be able to effectively read and interpret information, present numerical data in a resourceful manner, and skillfully gather and analyze information
  • Must be able to prioritize and plan work activities as to use time efficiently
  • Must be organized, accurate, thorough, and able to monitor work for quality
  • Must be dependable, able to follow instructions, respond to management direction, and must be able to improve performance through management feedback
