POSITION SUMMARY
The Diabetes Educator is responsible for providing direct patient education, information, and supporting self-management training to improve patient health outcomes in coordination with the medical management team. This position requires professional expertise in the diabetes diseases state and in motivational interviewing techniques. This is also a telehealth position. The standards of performance are upheld. Participate in training and development efforts of clinical staff to assure the appropriate management of plan members and the execution of well-being programming that is developed specifically for each client organization.
MINIMUM POSITION QUALIFICATIONS

Graduate of approved professional school.

RN license/RD license in state(s) of practice

Certified Diabetes Care and Education Specialist preferred
4
Pump experience preferred
5
Experience with education of patients with diabetes Type 1, Type 2, and gestational diabetes, group facilitation, and behavior change theory
6
Experience in adult learning theory
7
Knowledgeable about insulin delivery systems including insulin pumps, as well as continuous glucose monitoring systems
PREFERRED POSITION QUALIFICATIONS
Five to eight years’ clinical experience

Provide formal and informal diabetes education to clinic patients based on current knowledge and standards, participating in the ongoing development and revision of educational programs/materials

Document educational process accurately and participate in ongoing quality improvement projects for maintaining program recognition

Assist the medical staff in evaluating and adjusting treatment regimens based on established protocols, operating within professional scope of practice

Assist other clinic staff in providing effective, efficient care—including entering data, phone triage, performing vital signs/lab tests, compiling patient records

Maintain patient and clinic confidentiality

Ability to work closely and cooperatively with other skilled professionals on the multi-disciplinary diabetes care team and the clinical administration team to ensure an atmosphere of continual improvement in quality care, patient outcomes, and operational efficiency

Follow Vigilant Health’s operational policies and procedures as well as, local, state and federal requirements including regulatory and accrediting organizations such as but not limited to: CLIA, OSHA, CMS, Medicaid, State and Federal Governments as it pertains to the medical clinic. All responsibilities will conform to the professional and ethical standards of Vigilant Health as expressed in its Mission Statement and Compliance Program
