Job Summary:
Follows patients’ plan of care. Administers treatments, performs procedures and documents per facility policy.
Position Responsibilities:
Administers and monitors the effectiveness of medications/treatments ordered by physician and documents same. Assists in implementing the care plan by performing patient’s care as based upon identified needs consistent with plan of care and mutually agreed upon goals with patient/family/Case Manager. Evaluates the effectiveness of the plan of care and notifies Case Manager and Physician of changes in patient care. Responds to emergencies and reports to the Registered Nurse and Team Manager.
Complies with Virtua Standards of performance, federal and state regulation defined in nursing policy, procedure and standard operating procedures. Demonstrates technical competence in administering care including the use of equipment and supplies.
Participates in patient satisfaction program creating a supportive caring environment. Responds to patient/family needs and to customer concerns as soon as possible with compassion and understanding.
Practices universal precautions and adheres to agency policy with regard to the spread of infections. Actively participates in performance improvement initiatives and safety programs defined by nursing leadership.
Educates patient/resident and family/responsible party regarding individual care, needs and safety issues, etc. Maintains competency as defined by Nursing department goals and objectives.
Position Qualifications Required:
Required Experience:
One year previous experience.
Required Education:
Graduate of accredited LPN program
Training / Certification / Licensure:
NJ LPN license
Must have valid driver’s license and personal vehicle.
