Job Summary:
Join our team as a Home Care Coordinator where you will be an essential part of ensuring seamless coordination and delivery of care services to our elderly clients. As a Home Care Coordinator, you will be responsible for overseeing the administrative aspects of patient care, facilitating communication between Clients, our agency, our caregiving staff, and ensuring that patients receive the support they need. This role offers a fulfilling opportunity to contribute to the well-being of individuals in need.
Duties:
- Providing administrative support by scheduling appointments, coordinating referrals, and maintaining patient records.
- Collaborate with Insurance providers, caaregivers and other professionals to ensure comprehensive care plans are in place for each patient
- Communicate with patients to explain treatment plans, answer inquiries, and address concerns
- Assist in managing medical office operations by organizing files, ordering supplies, and ensuring a clean and welcoming environment
- Utilize knowledge of medical terminology to accurately document patient information and assist in medical coding
- Coordinate with insurance companies to verify coverage, obtain authorizations, and process claims efficiently
- Support the coordination of services for patients transitioning between different levels of care or healthcare facilities
Qualifications:
- Experience in Homecare administrative support is necessary.
- Proficiency in office software applications and electronic health records systems
- Familiarity with medical terminology and procedures commonly used in a healthcare or dental office setting
- Strong organizational skills with the ability to prioritize tasks effectively
- Excellent communication skills to interact with patients, healthcare providers, and insurance representatives
- Ability to work collaboratively in a fast-paced environment while maintaining attention to detail
