What are the responsibilities and job description for the IBCCP Community Navigator position at VNA Health Care?
Primary Functions:
1. Partners with local organizations for outreach events, education, and policy change.
2. Perform community environmental assessments and collect data for analysis.
3. Identify and decrease structural barriers to care in the community.
4. Establish and coordination of available clinical and support services and resources for women enrolled in the program who have been identified as needing services.
5. Provide culturally and linguistically appropriate breast health education.
6. Educate providers about IBCCP and their role in recommending screening.
7. Assist clients with problem-solving barriers to address health conditions by identifying, connecting to and navigating needed community and medical system services.
8. Assists clients with completing necessary forms.
9. Establish a trusting relationship and continuous candid communication with clients and client’s family support system.
10. Advocates for individual and community health equity.
11. Coordinates patient care and communication among VNA primary care provider, other VNA services, other specialty services, community resources and volunteer based organizations. Communicates with these members on a regular basis for progress reports.
12. Collaborate with patients and/or family members, providers, clinic staff and other relevant agencies to assure appropriateness of services that meet the patients’ needs and ensure desired outcomes.
13. Provide support, follow up and education.
14. Collaborates with the Communications team for the creation patient education, resource information/directories including local and national information, treatment centers, websites, Facebook pages, etc.
15. Conducts community outreach activities to include providing information and education at community events, speaking engagements, health fairs, community gatherings and other opportunities.
16. Assists with the collection and reporting of information and data for reports, funders, and grants pertaining to the IBCCP patient navigation program.
17. Makes regular telephone contact with patient to provide follow-up and/or assists with further care coordination.
18. Completes timely, current and accurate documentation for all patient care activities. Follows organization documentation guidelines including electronic and paper processes.
Qualifications:
1. Bachelor’s degree in social work or related field required. Masters of Public Health or Registered Nurse preferred.
2. Experience: IBCCP or oncology experience preferred, preferably with Community Health experience.
3. High degree of sensitivity to the rehabilitation needs of patients, their families, and the ability to establish constructive working relationships with them.
Prior case management experience in an acute, outpatient, or insurance setting beneficial
5. Excellent interpersonal and communication skills.
6. Bilingual Spanish preferred.
7. Proficient computer skills required.
8. Strong data analytical skills preferred.
9. EPIC experience preferred.
10. Must possess a valid driver’s license and have access to an operational vehicle to transports self to various locations throughout the VNA service area.
