About the Job
- Review medication orders for drug therapy and evaluate appropriateness of use, dose, dosage form, regimen, therapeutic duplication, and drug interactions
- Provide pharmacokinetic/anticoagulation dosing
- Participate in the provision of pharmaceutical care through clinical services, including medication reconciliation and discharge medication education
- Pharmaceutical compounding oversight, including USP regulatory/compliance guidelines (USP 797 and USP 800 focused)
