Job Requirements
- Fluency in English
- Spanish-speaking is preferred but not required
- Certified Medical Assistant with excellent communication skills and the ability to learn about Vytalize’s patient care services
- Experience working in a call center as a customer service representative is a plus
- Ability to simplify information and communicate effectively
- Ability to make a high volume of outbound calls efficiently
- Charismatic and positive personality with an ability to build rapport with elderly patients and get them excited about preventive health care
- Experience working with senior patients a plus; familiarity with Medicare and senior care service

Job Description
As a Patient Advocate / Program Enrollment Representative, you will be the first point of contact to our patients to introduce Vytalize’s care management programs and help them enroll. You will be contacting our eligible Medicare patients over the phone to educate them about our expanded care services.
Qualified candidates need to maintain a positive attitude and demonstrate patience, compassion and professionalism with elderly patients. You will be the face of our company to these patients therefore you will need to be comfortable making a high-volume of calls on a daily basis while representing the company in the best possible manner.
As a Patient Advocate, you will be expected to provide a high level of service when introducing Vytalize’s programs and when helping Medicare patients enroll. You must have a clear, professional and calm tone and demeanor over the phone with the ability to be friendly, courteous, and helpful. This role requires patience as our patients are older and dealing with chronic conditions.
Vytalize Health’s Care Programs Include:
- Chronic Care Management (CCM) services
- Behavioral Health Integration (BHI) services
- Remote Patient Monitoring (RPM) services
- House Calls
The role may evolve over time depending on the ability of the candidate; this will include focusing on enrolling new patients into programs, to eventually becoming a member of the care management team under the supervision of a Vytalize Health Registered Nurse and Medical Director. The goal is to meet the needs of patients with chronic conditions while promoting quality and cost-effective outcomes.
- Serve as the contact point and informational resource for patients, care team, family/caregiver(s), payers, and community resources. This involves reaching out to patients and enrolling them in CCM, RPM, and Behavioral Health Integration (BHI) services from a remote setting.
- Identify care needs for patients with high-risk conditions and communicate daily to the care team.
- Provide ongoing contact with patients as needed.
- As directed by the care team, educate patients and family/caregiver(s) about relevant information or community resources.
- Explain and prepare patients for visits and assist providers to deliver quality healthcare.
- Assemble and maintain medical supplies, instruments and other materials
- Verify patients' information, upkeep information and medical records accurate in the EHR
- Scheduling appointments on behalf of Medicare patients.
- Authorizing prescription refills as directed by the care team.
Performance Requirements
- Ability to work productively, either independently or with other individuals
- Highly organized with the ability to keep accurate notes and records
- Capability to effectively communicate with patients, providers, family members to implement successful care plans and strategies
- Demonstrate a positive attitude and respectful, professional customer service
- Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations
- Knowledge of healthcare practices, insurance and billing
Salary: $14.00 to $20.00 / hour
