Job Summary:
The Direct Report Professional is a non-exempt level role that provides hands on mentoring, coaching and support to individuals to gain knowledge, skill, and experience to control their life and environment. The Direct Support Professional encourages individuals to push their limits and explore things in their lives that are important to them. Through the introduction of new and novel experiences, the Direct Support Professional assists the person to maximize their potential and enjoy the life of their choosing.
Duties and Responsibilities:

Administration:
- Communicates and interacts positively and effectively with each person, visitors, family members, peers, and supervisors.
- Daily enters hours worked into the payroll and LTSS time tracking systems
- Daily enters documentation into Therap ISP, Health Tracking (including eMAR), Time Tracking and GER modules for individuals served
- Participates in the individual Person Centered Plan process by completion of the My PCP Worksheet with individuals supported, attending PCP meetings as requested, and reporting observations of individual interest regularly
- Attends mandatory meetings, including in-services, supervisory meetings, staff meetings and to other meetings as scheduled.
- Accurately documents individual and household expenses and submits receipts to supervisor monthly, as assigned.
- Monitors the safety and health precautions compliance according to all governing bodies’ policies and procedures.
- Comply with all policies, procedures, and requirements necessary to perform the functions of the position.
- Timely and accurately submit employee and individual Incident Reports and include all relevant documentation regulatory bodies require.
- Performs all other duties as assigned
Site Management
- Completes required safety drills, review of documentation of drill, and addresses all areas for improvement
- Reports safety and maintenance concerns to Coordinator timely
- Assures adequate food and household supplies are in place through purchase or report to Coordinator (as assigned)
- Reorders medications as needed
Day & Individual Supports
- Provides opportunities daily for individuals to explore self-advocacy, choice, opportunity, and learning within the home and community.
- Supports the development of meaningful relationships and natural supports in the community.
- Assists individuals to maintain maximum health potential through
- support of healthy choices

- attendance at a medical appointments (as assigned)
- use of adaptive and durable medical equipment, including mechanical lifts, shower chairs, wheelchairs, hospital beds, blood pressure cuffs, feeding equipment, etc.
- Reports any health, behavior or safety concerns to Coordinator and/or nurse.
- Assures the dignity of each individual is supported through
- Assuring privacy during performance of personal care and hygiene duties
- Honoring requests for private conversations
- Not sharing individual private or sensitive information unnecessarily or in an open setting.
- Maintaining positive interactions and support for emotional support needs
- Supports the individual to complete all personal care, hygiene, mealtime (including cooking, preparing and feeding), eating, and mobility tasks as independently as possible.
- Enters all medical and support documentation into Therap, including calling nurse to report outcome of appointment(s) and any new orders.
Vehicle Operation
- Adheres to all We Achieve, local, and state policies, procedures and laws regarding the safe operation of both agency and personal vehicles
- Completes vehicle safety checks and reports using the assigned system each time a agency vehicle is used.
- Completes accurate and timely documentation of the operation of agency vehicle when in use.
- Assures vehicle is clear of all trash, debris and spills after each use.
- Completes car wash as assigned.
- Assures vehicle consistently always has at least ¼ tank of fuel available at all times.
Human Resources Management
- Maintains current training certification for all agency, DDA and Person-Centered Plan trainings
- Follows all organizational policies
Qualifications
Required Qualifications:
- Fully Vaccinated against COVID-19
- High School Diploma
- Valid Driver’s License
- Possess a strong service orientation, including active listening, critical thinking skills, and close monitoring of individuals supported.
- Comfortable in an environment where individuals supported may not be able to perform essential life duties.
- Ability to work professionally and discretely with confidential information.
- High level of personal and professional integrity.
- Ability to communicate clearly and concisely in a verbal and written manner.
- Demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
- Ability to use computer applications to fulfill job duties, including the use of individual documentation system, Microsoft Word, assigned payroll systems, LTSS (as assigned) and vehicle inspection reporting system
Physical Requirements:
- Prolonged standing, leaning, walking, bending, and lifting periods.
- Prolonged periods are sitting at a desk and working on a computer.
- Ability to lift, push, and pull up to 50 pounds.
- Ability to operate durable medical equipment.
- Ability to push a wheelchair and occupant with combined weight up to 200 pounds, on occasion.
- Ability to detect unsafe situations by sight or sound and react quickly to intervene.
- Must maintain reliable transportation to ensure the ability to travel to various sites and meetings in the community.
