What are the responsibilities and job description for the Community Health Worker position at Welcome Health?

Welcome Health is an innovative medical group that delivers home-based primary care directly to older adults in the greater Los Angeles and Orange County area. Founded in the science and art of geriatric medicine, the team at Welcome Health believes that the best primary care is anchored on what matters most to each person. Our care model recognizes the unique needs of older adults.

In addition to preventative care and chronic disease management, we focus on aging-related issues that significantly impact the quality of life of our seniors, such as mobility limitations, cognitive impairment, incontinence issues, and polypharmacy. In our physician-led care model, community health workers partner closely with physicians and other team members to implement the care plan for each patient within a shared patient panel. Because we care for home-bound patients who otherwise have difficulty getting to the doctor, the majority of the work associated with this position will occur in the field, caring for patients in their homes. When not in a patient’s home, work will be done remotely. What you will do?

  • 50-70% of the position entails conducting the “Welcome to Welcome” meeting with the patient at their home to onboard the patient to Welcome Health, gather forms, and complete preliminary questionnaires.
  • Support and advocate for patients by helping them navigate their healthcare through various providers, health systems, payers, and community resources to achieve the patient’s goals of care.
  • Facilitate the communication between patients/caregivers, providers, health systems, payers, etc. to support the patient’s care
  • Encourage and motivate patients to make progress toward their stated goals
  • Help patients understand and utilize the health plan benefits available to them
  • Build relationships with community resources and maintain their contact lists to support patient needs
  • Help patients navigate programs designed to alleviate challenges related to social determinants of health (SDOH)
  • Discuss the benefits of Welcome Health with potential patients, caregivers, community resource groups, and other groups as directed by Welcome Health leadership
  • Foster a trusting and open relationship with patients and their caregivers
  • Guide patients through the process of referrals and authorizations
  • Maintain and update documentation for patient encounters
  • Support the improvement of Welcome Health processes and operations
  • Adhere to regulations, policies and other compliance guidance
  • Participate in patient acquisition activities as needed
  • Perform other duties as directed

What we are looking for?

  • Bachelor’s degree in social work or related field, or equivalent experience
  • Experience navigating health plans, provider groups, EMR and other healthcare stakeholders
  • Superb customer service and organization skills
  • Able to receive and maintain CPR/BLS certification
  • Experience developing relationships with community organizations, non-profits, provider groups and other key stakeholders
  • Experience using Athena is preferred
  • Thrives in a fast paced, ever-changing environment, and excited to help build a great organization
  • Comfortable with ambiguity and can work independently
  • Fluency in a second language is highly desirable; Spanish fluency preferred
  • US work authorization
  • Must be comfortable working in a patient’s private residence, which may have allergens or odors such as pet dander or tobacco smoke
  • Valid Drivers License, 2 years of driving experience and transportation to facilitate mobile work

Job Type: Full-time

Pay: $40,000.00 - $70,000.00 per year
