As a Case Manager for our Home Health team, you will:
Travels to complete visits for each assignment
Instructs, supervises, and evaluates home health aides, visits nurses, and licensed practical nurses as applicable.
- Monitors for and ensures visits and re-certifications are completed per regulatory compliance.
- Participates in weekly Home Health Medicare meetings, providing pertinent updates on current client status.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments prescribed by physicians.
- Counsels clients and families about nursing and related needs.
- Provides health care instructions to clients as appropriate per assessment and plan of care.
- Assists clients with activities of daily living and facilitates client efforts toward self-sufficiency and optional comfort care.

Experience
Required- Valid driver’s license, reliable transportation, valid insurance, and clean driving record
- Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen
- Active RN license in the state of Iowa
- A minimum of 2 years of clinical experience is required
- Bachelor's degree (BSN) is highly preferred
- One year of home care experience is preferred
