The Care Coordinator advocates on behalf of the client, making linkages/referrals for services, coordinate services and collaborating with other agencies for continuity of care for children exposed to sexual assault, domestic violence, dating violence and/or stalking and their non-abusing parent.

Essential Duties And Responsibilities
- Provide direct services to those identified as children exposed to violence and/or their non-abusing parent
- Coordinate services provided by ancillary agencies: linkages and referrals.
- Conducts Needs Assessments
- Prepares service plan documents
- Development of the discharge plan in coordination with other treatment team members
- Provide sessions with respect to case management and Service Plan development
- Responsible for maintaining the clients chart in compliance with licensing, CARF, Medicaid, and other regulatory standards.
- Attend and participate in project related training, interdisciplinary team meetings, staff meeting and community task forces
- Perform other duties as assigned by the Director/Supervisor
- Understanding and application of federal, state and local confidentiality rules and professional standards of conduct
Essential Qualifications:
- Should be CAP, CPRS, or CPMH eligible. If not certified, must have ability to obtain the credential within 2 years of employment Preference will be given to those candidates who are currently certified
- Must be CPR/First Aid certified within 6 months of employment and every two years thereafter.
- Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
- Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
- Ability to meet requirements of Monroe County Sheriffs Office background check
- CPR Certification, First Aid Certification and an annual Tuberculosis test.
- Ability to provide an Affidavit of Good Moral Character
- Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
