Experience/ Requirements:

The Case Manager, under the direction of the Director of Case Management, serves as an interdisciplinary team member to coordinate the delivery of healthcare services leading to quality outcomes for the inpatient, ambulatory surgery and observation population with the limits of the financial and social resources available in a manner reflective of the Hospital’s philosophy and objective for maintaining and for acting as a liaison between the Hospital Administration and the staff.
- A minimum of three years of clinical experience. Must have Case Management experience.
- Bachelor’s Degree in Nursing preferred
- Current PA RN License
- BLS certification is required and must be completed prior to the end of the orientation period
