Position Summary:
This role is responsible for monitoring quality improvement through data collection and analysis. Will require a high level of understanding of organization priorities, structures and goals. This position will require implementation of process changes; and communicating results with physicians and staff.
Position Requirement:
Formal Education / Training:
Earned a nursing diploma or degree from a college or university
Must be currently licensed by the State of Tennessee
Workplace Experience:
At least three years of experience in medical office environment
Equipment and Skills Training:
Demonstrated clinical knowledge base in medical office setting. Demonstrated organization, problem solving and critical thinking skills. Able to perform independently and in team situations. Demonstrated skill in effective communication skills.
Physical Environment:
Office and Clinical environment. Possible exposure to airborne pathogens.
Physical Effort:
Able to use the telephone and computer for extended periods of time. Able to walk/stand, up to 4 hours/day.
Clear legible handwriting. Clear understandable voice
Key Results:
Coordinates the evaluation process of the defined patient population. Collaborates with other team members and completes and documents select portions of the evaluation process.
Assists in identification of appropriate patient population through chart review, data analysis and direct communication with providers and teams.
Maintains comprehensive documentation of evaluation process for presentation to the multidisciplinary care planning conference, making recommendations for standardized vs. enhance care.
Assesses patients,(clinically for adherence, social factors), reviews and actively participates in development of protocols, care pathways and continuum of care planning with providers and front line staff, overseeing adherence to agreed upon plan of care.
Communicates information to patients and/or caregivers regarding plan of care throughout the continuum.
Per standards of practice demonstrates sound clinical judgment and disease expertise to assess and resolve problems in collaboration with the multidisciplinary team.
Per departmental area specifications, participates/leads meetings with clinical staff, faculty, and the clinical manager to review and discuss patient cases in order to refine existing processes.
Implements performance improvement action plans and collects data for analysis of further improvement opportunities.
Clearly and efficiently communicates significant information with team members and staff; including: appropriate handoff of information to involved personnel, following established guidelines and resolution procedures.
Develops and achieves personal and professional goals, while also contributing to the overall departmental goals.
Participates in on-going quality improvement activities such as data collection, outcomes management, readmission surveillance.
Evaluation of performance against established benchmarks for cost, LOS, and quality outcomes.
