What would my job responsibilities be?
Management Duties:
- Foster a dynamic relationship with the Executive Director and Clinical Directors, addressing legal issues and ensuring smooth program functioning.
- Engage in cross-training with other agency program directors to expand knowledge and skills.
- Work closely with YSFS senior leadership to review and refine the vision, mission, and goals of the organization, fostering cross-program collaboration.
- Ensure the successful delivery of Transitional Living Program operations, overseeing case load management, client facilitation, and interactions.
- Monitor and ensure contract and HIPAA compliance, including data collection, file audits, and program reporting requirements.
- Manage external relations with individuals, institutions, agencies, funding and referral sources, and community groups.
- Collaborate with the Human Resource Director and Executive Director to develop, implement, review, and revise operating policies and procedures for the Transitional Living Program.
- Oversee the development and implementation of annual operating plans and budgets.
- Responsible for the development and writing of funding proposals, as assigned.
- Participate in the hire, training, and supervision of Transitional Living Program staff, including volunteers and interns, ensuring clear understanding of job requirements and obligations.
- Set solid, attainable goals and hold performance reviews against these goals, encouraging learning and educational opportunities, both professionally and personally.
Program Specific Duties:
- Exercise discretion and independent judgment while working under minimal supervision.
- Assess housing barriers of youth and young families experiencing homelessness to determine housing and service needs.
- Provide comprehensive case management services, including client assessments, self-sufficiency plan development and implementation, coordination, review, and discharge and after-care planning.
- Facilitate leadership opportunities and activities to support clients in achieving their personal goals, utilizing individual contacts, group processes, and participation in the Workforce Development program.
- Deliver individualized life skills instruction and conduct regular life skills groups covering various topics for TLP clients.
- Assist participants in developing strength-based/solution-focused individualized goal and action plans, promoting permanent housing and self-sufficiency. Develop an effective referral network for ongoing support.
- Identify participant strengths and barriers to stability, assist in reducing barriers, and facilitate connections to resources and services.
- Maintain accurate daily logs, monthly outcome reports, and comprehensive case files for each participant, ensuring proper file maintenance procedures.
- Oversee facility and grounds maintenance, vehicle and equipment upkeep, and associated record-keeping activities.
- Provide transportation for clients as necessary, limited to housing and job searches, and occasional visits to relevant social service agencies.
- Collect and report program data, including HMIS reporting and required data for funders.
- Provide direct client care and fulfill daily program functions as needed.
What are the qualifications and experience I need to become a Program Manager for - Transitional Living Program?
- Experience overseeing and managing a transitional living program, or similar initiatives focused on empowering individuals in their journey towards independent living.
- Experience with homeless and at-risk youth preferred.
- Experience with residential services related to youth homelessness preferred.
- Case management experience required.
- Proficient with computer software skills including Microsoft Windows, Excel, and Word.
- Excellent oral communication skills, particularly listening, mediation, and writing skills.
- The ability to establish and set appropriate limits with people served to help them gain skills and confidence.
- The candidate must be able to develop strong working relationships with the client population, be able to relate to individuals making significant life decisions and be supportive of individuals’ increasing independence.
- Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths based” case management.
- Strong project management and planning and budgeting skills
- Ability to adapt to shifting priorities.
- Ability to relate in cross-cultural environments.
- Must be able to drive company vehicles.
- Exceptional attention to detail and time management skills.
- Must be over 21 years of age to meet the minimum eligibility requirements of our automobile insurance carrier.
What are the minimum education requirements?
- Master’s degree in counseling, social work, psychology, or related field preferred
- OR bachelor’s degree in human services preferred AND at least two years related experience, including experience with in-crisis, runaway and homeless youth and experience preparing youth to transition into independence.
