Job Description Job Description The Parent Coach/Counselor will be responsible for outreach, education, counseling and care coordination services for families with mothers who are pregnant or have young children and who either have a past history of substance use or who are at risk for future use. Counselor will perform assessments and develop service plans in collaboration with the family, provide brief counseling and intervention services, provide individual and group parenting education and one on one parent coaching, provide care coordination in collaboration with the Recovery Coach, and other duties to ensure the health and wellness of the family. Counselor must have the ability to successfully engage and retain participants and to motivate them to adopt healthy behavioral changes. The chosen candidate will exhibit a genuine passion for serving at-risk women and families and will be comfortable actively engaging families in the home and community setting.
