Job Description Job Description BJK Peoples Health Centers is a Certified Community Behavioral Health Organization that provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. BJK Peoples Health Centers operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth. Summary: People's Health Centers currently has an opportunity available for a Community Support Specialist (CSS) in our Adult Services Department. This position is responsible for providing activities and interactions that enable an individual at clear risk of hospitalization, or who was discharged from inpatient treatment to maintain an adjustment to living in the community.

Develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed. Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. Manages the assignment of caregivers. Responsible for and oversees the delivery of care to all patients served by the location. Receives case referrals. Reviews available patient information related to the case, including disciplines required, to determine home health or hospice needs. Accountable to ensure patients meet admission criteria and make the decision to admit patients to service. Assigns appropriate clinicians to a case, as needed.
