Job Description: Job Title: RN- Home Health Home Department: Home Health Reports To: Director of Home Health Direct Reports: None FLSA Classification: Non-Exempt Position Summary The RN is responsible for assessing, planning, coordinating, implementing and documenting care for an assigned caseload of patients in the Home Health department. Develops and implements a home health plan that meets each patient's specific needs and is in compliance with regulating agencies, federal and state regulations, reimbursement guidelines and agency policies and procedures. General Duties, Tasks and Responsibilities •Performs accurate and complete assessment of patients; performs initial admission assessment; coordinates and ensures completion of interdisciplinary screenings and assessments; assesses and reassesses pain; assessments are age, culture and psychosocial appropriate; recommends appropriate referrals to other disciplines/community resources. •Formulates a plan of care based on patient needs
