Old Colony Hospice, Inc. is a non-profit, Medicare certified, accredited health care organization that provides interdisciplinary services to meet the physical, spiritual and psychosocial needs of patients with advanced disease and their families/loved ones. Old Colony Hospice has provided hospice care since 1979 within 36 cities and towns located in Bristol, Plymouth and Norfolk counties. Old Colony Hospice is a free-standing hospice with contracts in many skilled nursing and assisted living facilities, as well as five acute-care hospitals. Services are provided through a core staff of nurses, social workers, counselors, volunteers and home health aides. Our medical director oversees the team and provides consultation as necessary for pain and symptom management. Physical, occupational, nutritional and speech therapy are provided through contracts with area agencies.
