Job Summary: This pharmacist will operate under our ambulatory care pharmacy services umbrella and primarily provide direct patient care within our ambulatory cardiology clinic. This pharmacist will work directly with providers and support staff to optimize medication therapy, provide patient/provider/nurse education, and assist with transitions of care management. This pharmacist will also work closely with our specialty/retail pharmacy division to provide medication oversight and monitoring in accordance with specialty medication requirements. Overview: Scope of Responsibilities: Provides vaccination services to Kinney patients within the store and at offsite locations. Responsible the processing, administration, and documentation that is consistent with State and Federal laws/regulations and Kinney Drugs policies and procedures as it relates to vaccinations. Vaccinations provided will include all vaccines allowed by the Prep Act and/or by State Law to patients ages 2+ in NY and 3+ in VT.
