Admissions Coordinator - Nursing Home jobs in Taylor, MI

The Director of Admissions is responsible for timely and accurate responses to referrals in order to bring patients into the building. The coordinator needs to ensure that all residents that are accepted are appropriately placed within the facility and staff is appropriately prepared to care for the admission. Although interdisciplinary effort is required, the Director of Admissions includes but is not limited to; response to referral inquiries, facility tours, sales calls, discharge planning, QAPI, harmonious workplace, customer service, assures resident safety, policies and procedures, federal and state compliance. The Director of Admissions must lead by example by promoting positive morale within their department and throughout the facility by smiling and greeting residents, families and all employees. Reports directly to the Administrator, does related work accordingly;

Duties include (but not limited to):

• Responding to referrals timely (within 15 minutes)
• Keeps team informed of new referrals through referral text group
• Verifies all insurances through PCC
• Obtains insurance authorizations
• Input all referral info into PCC
• Verifying med costs
• Following the red/yellow/green light form
• Reading referrals prior to bringing to DON/Social Service’s/Admin for approval
• Fill out the referral cover sheet
• State and national background checks for criminal history
• Room readiness
• Bed management
• Admission contract to be completed upon admission
• Fully executed admission contracts to be delivered to the business office within 48 hours
• Greats new patient by the door and shows them their new room
• Visits new admits morning after to make sure resident is acclimated prior to morning report
• Sends out BOD and EOD census reports
• Send admission notice to all staff once an admit is confirmed
• Pre-admission meeting
• Documents admission data throughout the month to complete the monthly admission reports
• Assigned room checks prior to morning meeting
• Attends morning meeting
• Assure resident safety
• Reports census at morning meeting
• Reports findings from room checks at morning meeting
• Attends QAPI
• Performs all job duties in a manner that ensures that confidential information and resident rights are protected at all times
• Participates in Manager on Duty program
• Participates in Night Manager program
• Participates in Dining Room Manager program
• Supports and promotes facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers.
