- Conduct assessments to determine client needs and develop individualized care plans
- Coordinate services and resources to meet the client's goals and objectives
- Provide medication administration assistance as needed
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care
- Maintain accurate records and documentation of client progress
- Facilitate communication between clients, caregivers, and service providers
