Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Job Summary: Performs a wide variety of clinical and distributive duties to help meet the pharmaceutical needs of the organization and its patients. Provides medication-related information to physicians, staff, and community. Serves as a liaison to all patient care areas with responsibility for outcomes including patient satisfaction and quality measures. Mentors and educates pharmacy staff, residents, and students in area(s) of practice. Coordinates delivery of pharmaceutical care within CaroMont Regional Medical Center Belmont by serving as the subject matter expert for order sets, patient care protocols, policy development / revision, and multidisciplinary rounds. Responds at patient bedside to selected medical emergencies throughout the facility to support pharmacotherapeutic selections and facilitate the prompt delivery of medication related patient care.
Qualifications: Bachelor’s degree in pharmacy or Doctor of Pharmacy degree from a recognized college or university required; 2 years pharmacist experience or completion of an accredited PGY1 residency program preferred; 6 months clinical experience in the hospital setting preferred; current NC pharmacist license required; Board Certification preferred. Excellent teamwork, communication, and teaching skills. Thorough knowledge of pharmacy terminology, procedures, and policies. Knowledge of quality measures. Considerable skill in performing pharmacy procedures; Considerable ability to communicate effectively with multidisciplinary team members; Maintains active BLS, ACLS, and PALS certification. Occasional exposure to hazardous chemicals.
