Job Title/Position: Registered Nurse Quality Assurance
Reports to: Administrator
Job Description Summary
The registered nurse is one who is licensed to practice nursing in the state of Ohio. The quality assurance manager interprets and implements quality assurance standards in home health care environment to ensure quality care to patients by performing the following duties. Must be flexible to accommodate completion of tasks, client needs and referrals. Position based at Gallipolis office.
Essential Job Functions/Responsibilities
1. Responsible for all start of care admission documentation review process. Accurately reviews all start of care admission documentation including OASIS face to face encounters initial nursing notes and plan of cares.
2. Reviews 100% of initial plan of cares and recertification plan of cares. Works with clinicians to accurately capture and identify patient goals and goal elements,
3. Maintains OASIS certification.
4. Responsible for 100% of OASIS reviews.
5. Works with field staff to provide education regarding OASIS documentation and plan of care development.
6. Monitors reports to assure timely completion of all Plan of Cares.
7. Working Knowledge of OASIS requirements and educates staffs on new developments and changes.
8. Working knowledge of home health rules and regulations including COP’s.
9. Working knowledge of face-to-face documentation. Works with providers to assure accurate and complete information is received.
10. Responsible for discharge OASIS review.
11. Work with Administrator and Assistant Administer to improve quality star ratings.
12. Special projects and other related duties as assigned.
Position Qualifications
1. Graduate of approved school of professional nursing.
2. Holds a current registered nurse license to practice nursing in the state of Ohio.
3. Prefer minimum of five years home health experience
4. Prefer OASIS certification.
5. Proficient with Microsoft Office products
