Share Care USA is looking for a mature and experienced Field Supervisor to run and manage a case load. This person should be mature, proactive, and wanting to make their clients succeed.
HCBS Client Supervisor:
1. Staff all shifts within the case load.
2. Reviewing all time sheets and progress notes for errors or corrections.
3. Assist with client financials as required such as but not limited to (collecting receipts timely, dropping off rent checks, explaining finances to clients, and teaching client about spending patterns).
4. Updating monthly client calendars
5. Meeting with Service Coordinator weekly
6. Attending team meetings as scheduled
7. Communicate with support coordinators when necessary in order to maintain current data on all clients.
8. Complete all individualized service plans for clients.
9. Participate in the planning of services for all clients.
10. Provide weekly / quarterly face-to-face visits with all clients as evidenced by signed face-to- face visit forms.
11. Provide bi weekly / monthly phone contacts with clients as evidenced by phone record logs.
12. The Field Supervisor will be on an On Call rotation. Must fill in as DSW if needed.
13. The Field Supervisor will need to live in region to accommodate emergency staffing requirement and rotating On Call.
14. All other assigned tasks.
