The Referral Coordinator is responsible for supporting all elements of referral and authorization to include validating patient's health plan eligibility and obtaining authorization for all services and procedures in office and external facilities. Contacting patients and scheduling into the Epic system. Contacting clinical staff to coordinate peer to peer calls between Physicians and the health plan as need
The Referral Coordinator will work as an educator, healthcare resource, and patient advocate to ensure patient care is coordinated and managed appropriately. He/she will refer patients to external physicians/specialist, serve as a liaison to the physician and coordinate care across the healthcare continuum.
