Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     Alphacare Support Coordination, LLC is located in Chalmette, LA. We are challenging and collaborative. The ideal candidate for this position will have the ability and drive to learn and show proven ability to provide excellent service to the individuals within our community. The Support Coordinator will be responsible for managing the day-to-day administrative needs as well as multi tasking to ensure that client files remain up to date.

    Support Coordinators are responsible for:

    • Completion of quarterly visits, generating required units, if applicable, observation and monitoring of monthly contracts.
    • Conducts and completes annual CPOC's according to regional office timelines.
    • Planning, scheduling, coordinating quarterly meetings with participants and providers.
    • Responding to consumer changes in needs, completing/submitting appropriate paperwork.
    • Researching, accessing and linking services.--Communicating and working with providers to ensure appropriate service provision.
    • Maintaining all responsibilities and requirements in regards to critical incidents.
    • Meeting with supervisor for weekly supervision, consultation, and participants needs.
    • Attending and participating in weekly staff meetings.
    • Attending and participating in required training.
    • Reliable transportation.
    • Must have organization skills and ability to prioritize.
    • Willingness to travel during the day to outlying parishes as required.
    • Must be able to multi-task and meet deadlines.
    • Must have excellent written and verbal communication skills
    • Must be computer literate and proficient in the use of Microsoft Office Software (i.e. Word, Excel)
    • Must be a TEAM PLAYER!
    • Employees are required to operate a computer, file and retrieve written documents, communication with others on the phone and in person.
    • Employees frequently walk, sit, use hands, lift and/or move lightweight items.
    • Experience in the following programs a plus but not required: OAAS, EPSDT and OCDD

    This is an in-office position.

    Salary: $33,000.00 - $38,000.00 per year
