Triage and Integration Specialist, Pittsfield

  • CSO is looking for a Triage and Integration Specialist to join our team in Springfield.

    This Triage and Integration Specialist will support full integration of CSO services for clients, including initial screening, assessments and triaging appropriately.

    Responsibilities

    • Provide skills training and assistance in areas of daily living activities including basic living and interactive skills. Assist in developing communication and conflict resolution skills.
    • Provide counseling and intensive support to consumers during psychiatric crises or periods of transition.
    • Assist consumers with identifying appropriate mental health, addiction and medical services and accessing services of choice.
    • Provide assistance with developing and maintaining a "natural support network" of family and friends.
    • Assist in locating acceptable housing, negotiating leases, and obtaining furniture and other necessary fixtures.
    • Complete NOMS, Quarterly Health Assessments and other required documentation in a timely manner. 
    • Collect and enter all demographic and insurance information and verify insurance.
    • Determine level of urgency: routine or crisis, and triage appropriately.
    • Make appropriate internal agency and program referrals for clients as needed, maintaining an in-depth knowledge of all CSO programs and referral sources.
    • Stay informed about payor requirements and managed care plans.
    • Act as liaison between CSO and community agencies and groups as appropriate.

    Qualifications

    Associate's or Bachelor's degree in a behavioral health discipline or related field; Good listening skills; ability to establish trusting relationships through compassion, empathy and insight, and ability to foster self-sufficiency in real-life situations using problem-solving and practical; Excellent oral and written communication skills; computer proficiency. Must be able to work flexible schedule which may include evening and weekend work, as required by client needs. Valid driver's license and reliable transportation required.
