LifeSpring Health Systems is seeking an experienced licensed family practice physician to work on an interdisciplinary team providing primary care to patients and families at our FQHC location in Jasper, IN. This position will provide supervision to other offices including Dale, Orleans, and the mobile unit. The physician will work to assist the patient in stabilizing any acute conditions, prioritizing care for chronic health conditions and engaging in preventative healthcare. Many of the patients seen in our practice have multiple comorbid health conditions. The successful candidate will be comfortable practicing autonomously, and be licensed or license-eligible in the State of Indiana. They will also need to be comfortable working with behavioral health providers, SUD, and MAT. The preferred candidate will have at least three years of experience providing primary care to individuals with chronic conditions.
