Family Assessment Worker & Data Specialist

  • GENERAL DESCRIPTION:

    The Family Assessment Worker/Data Specialist is responsible for initial identification, referral and coordination activities for high-risk families of newborns and for the input of program data. This position reports directly to the program supervisor.

    SUMMARY OF ESSENTIAL JOB FUNCTIONS:

    FAMILY ENROLLMENT SPECIALIST:
    • Conduct in hospital or clinic outreach or screening duties as outlined in the MOU. 
    • Establish and sustain partnerships within the community. 
    • Attend identified community coalition and service group meetings as assigned. 
    • Receive and process daily, referrals from hospital personnel as well as other agencies.
    • Maintain a referral organizational system. 
    • Schedule assessments meeting the needs of the family. 
    • Adhere to HFA standards regarding contacts, time frames, assessment rates etc. 
    • Complete quarterly FAW reports. 
    • Offer and administer the FROG Assessment virtually or in person, with all eligible families to determine risk level and need for services.
    • Ensure referral to the Family Support Specialist for continued follow-up and support services.
    • Creates electronic family file, uploads to FSS’s folders. 
    • Maintain client records in accordance with program and funding source requirements.
    • Meet weekly with Family Assessment Supervisor to discuss referrals and narratives.
    • Attend and participate in all scheduled Healthy Families seminars, conferences and in-service trainings and Parent Events.
    • Make DCS referrals as appropriate.
    • Provide community presentations and community outreach activities.
    • Maintain professional positive relationship with hospital staff and other collaborative agencies.
    • Will assist program as assigned/needed when enrollment openings are limited. 
    • Perform QA tasks as assigned. 
    • Perform other related duties as assigned.
    DATA SPECIALIST:
    • Assist with maintaining Healthy Families La Paz County data in the ETO System that includes entering demographic information, developmental evaluations, parent child interaction evaluations, safety information, home visit notes, goals, caseload tracking, and funding tracking.Enroll new families into the ETO system.
    • Enter data accurately and within funder time requirements.
    • Run ETO reports and communicate to supervisor and staff data needs. 
    • Maintain and protect the confidentiality of the data and uphold any proprietary rights associated with hardware/software
    • Ensure that the data entry is completed for all families closing out of services.  Move EFF file to “closed” folder. 
    • Participate in CQI activities as determined by the Program Director
    • Provide administrative support to Program Supervisor and program staff as it pertains to the ETO Data reporting system. 
    • Provide administrative support to Program Director as it pertains to program quality assurance and quarterly reporting. 

    REQUIREMENTS:
    • Minimum of a high school diploma or equivalent required. Bachelor’s degree or higher preferred. 
    • Location: must be able to provide direct service to the designated client base in the outlined service area (La Paz County).
    • Maintain a functional and safe home working space with adequate internet connection for the job.
    • Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage and dress code.
    • Meet program/department’s performance and productivity standards.
    • Attend required agency and program/department meetings.
    • Adhere to the schedule agreed upon with the supervisor, to include occasional evenings and weekend hours.  
    • Follow and model CFR’s core values.
    • Participate in PQI initiatives.
    • Must be eligible and able to obtain a Level one fingerprint card.
    • Must have a valid Arizona driver’s license, vehicle and meet agency liability insurance requirements.
    • Ability to communicate effectively both orally and in writing. 
    • Knowledge of and sensitivity to individual/cultural differences. 
    • Ability to establish trusting relationships and interact effectively with staff, families and partners.  
    ABILITY REQUIREMENTS:
    • Sitting for extended periods of time daily; standing and walking short distances.
    • Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
    • Ability to use a variety of office equipment and machines as referenced.
    • Normal manual dexterity and eye-hand coordination required.
    • Repeated hand-wrist movement required.

    The nature of the position involves a fast paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice and the position requires quick response time and flexibility.
