We are looking for a nurse to join our team!

The main focus of the role will be on completing the following tasks:

• Patient communications via patient portal and telephone
• Medication refill requests
• Provider orders (referrals, prior authorizations, etc)
• Other patient inquiries
• Triage via electronic messaging and telephone communications
• Assisting with direct patient care on an as needed basis

Experience:

• Nursing: 1 year (Preferred)
• Phlebotomy: 1 year (Preferred)
• Medication Reconciliation: 1 year (Preferred)
• CLIA Waived Testing (flu swab, A1C, Urine dipstick, etc): 1 year (Preferred)
• Chronic Care Management: 1 year (Preferred)
• Patient Triage: 1 year (Preferred)
• Referral/Diagnostic Ordering: 1 year (Preferred)

License/Certification:

• LPN or LVN (or graduating from nursing school soon) (Required)
