    The Case Manager will function as a multidisciplinary team member providing case management and service coordination as assigned.

    Position Summary


    Essential Duties and Responsibilities

    The following duties are normal for this position; however, these are not to be construed as exclusive or all-inclusive. Other program specific duties may be required and assigned.

    • Assess and determine program eligibility (functional and financial)
    • Assess and identify individual service needs
    • Develop and monitor consumer driven service plans, including six month reviews and updates
    • Prepare and complete required documentation with proficiency and in a timely manner
    • Meet client and collateral contact standards as set forth by program standards
    • Provide outreach services, as applicable
    • Manage crisis intervention services, as applicable
    • Work collaboratively with other professionals, including but not limited to, school personnel, hospital/clinic staff, social workers, law enforcement agencies, service providers, etc.
    • Serve as back up support to other case manager within the department
    • Attend and participate in advisory committee meetings as assigned
    • Maintain professional working relationship with clients, outside agencies, county department, the general public and supervisors and co-workers
    • Maintain confidentiality and adhere to HIPAA rules and regulations
    • Follow Department polices/procedures and adhere to the Clark County Employee Handbook and Administrative Manual
    • Maintain professional appearance and demeanor

    Salary Information


    Pay Range - $27.05 - $34.30, Health Insurance, Paid Time Off, Dental Insurance, Vision Insurance, Retirement Plan, Flexible Spending Account, Employee Assistance Program

    Job Details


    Full-Time, Monday-Friday position, background check will be done prior to hire.

    Qualifications

    • A Bachelor’s Degree in Social Work or a Human Services related field is required.
    • A minimum of two years of experience working with individuals with developmental/intellectual disabilities, mental illness and/or substance use disorders is preferred.
    • An equivalent combination of education and experience will be considered.
    • If applicable, the candidate must obtain and maintain certification to administer the Children’s Long Term Support Functional Screen and/or the Mental Health/AODA Functional Screen within one (1) month of hire.
    • Candidate must possess a valid driver’s license and own a reliable vehicle.
