Case Manager: Behavioral Health

Key Responsibilities:
- Provide Individualized Support: Conduct comprehensive assessments to identify clients' needs, strengths, and goals. Develop individualized service plans based on their unique circumstances and collaborate with clients to establish achievable objectives. Ensures a harm reduction approach while leveraging motivational interviewing skills to support the person served.
- Community Integration: Assist clients in accessing and utilizing community resources, services, and supports to enhance their quality of life. Facilitate connections with housing, employment, education, healthcare, and other community-based programs to address factors related to social determinants of health.
- Skill-Building: Teach and support individuals in developing essential life skills such as problem-solving, decision-making, budgeting, time management, and socialization. Offer guidance and encouragement to empower clients in managing daily activities and maintaining stability.
- Crisis Intervention: Respond promptly and effectively to crisis situations, providing appropriate support, de-escalation techniques, and linkage to emergency services when necessary. Collaborate with emergency responders, mental health professionals, and other relevant parties to ensure the safety and well-being of clients.
- Advocacy: Serve as an advocate for clients' rights, ensuring their voices are heard and respected. Collaborate with other service providers, community organizations, and stakeholders to advocate for improved mental health services and support systems.
- Documentation and Reporting: Maintain accurate and up-to-date records of client progress, service plans, interventions, and outcomes. Complete required documentation in a timely manner according to Front Steps policy, CARF, and OhioMHAS standards while adhering to confidentiality and data privacy guidelines.
- Collaborative Teamwork: Work closely with a multidisciplinary team of internal and external professionals, including psychiatrists, therapists, case managers, and peer support specialists. Participate in team meetings, case consultations, and coordination of care to ensure a holistic approach to client care.
Qualifications:
- Associate's degree in human services field required (Bachelor’s degree in a related field preferred).
- Current state licensure or certification as a social worker, counselor, or related professional. LSW, CDCA, or comparable behavioral health credential.
- Minimum of 2 years of experience working with people in a social services setting.
- Valid driver's license and reliable transportation for local travel as needed.

Working Conditions:
- Standard daytime hours of operations.
- Work is normally performed in a typical interior/office/clinical work environment.
- Requires periods of sitting, standing, telephone, and computer work.
- Hearing: adequate to hear clients or patients in person, over the telephone or through virtual technology.
- Speaking: adequate to speak to clients or patients in person, over the telephone or through virtual technology.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 25 pounds, unassisted.
- Possible exposure to blood borne pathogens while performing job duties.
- Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
