Case Managers must have a bachelor’s degree in a human service related field including but not limited to psychology, education, rehabilitation counseling, or counseling from an accredited institution; and one year of paid experience in a human service field providing direct consumer services or case management or
A bachelor’s or master’s degree in social work from a social work program accredited by the Council on Social Work Education;
Thirty hours of graduated level course credit in a human services related field may be substituted for the one year of required paid experience. Experience may be obtained before or after completion of the degree or obtaining licensure.
Responsibilities:
Provide Support Coordination services to families, with children ages 0-3 years, who receive Early Steps Services.
Facilitate and monitor provision of Early Steps Services
Maintain Early Steps Records
Document ongoing services.
Develop Individual Family Service Plans.
Conduct monthly contact with parents or caregivers.
Schedule Annual assessments.
Assist families with choosing providers.
Collect relevant reports.
Conduct quarterly and annual meetings.
Make community resource referrals.

Pay: $14.00 - $15.00 per hour
