Major Duties and Responsibilities:
· To be knowledgeable of and follow all agency policies and procedures including the agency’s Personnel Manual, Residential Services Manual, and Program Philosophy.
· Implement individual ISP’S and ensure persons are trained annually and documented on Client Specifics.
· Develop annual Task Analysis/Short term goals in accordance with ISP and submit to MCO as applicable.
· Complete quarterly progress summaries.
· Complete Notice of Change as applicable, per MCO standards
· Submit to MCO when individuals go out of state with AFL’s, as applicable.
· Participate in treatment team meetings, behavior meetings as well as other meetings in regards to the individual as needed/requested.
· Monthly visits with AFL providers and individuals
· Review bi- monthly documentation
· Maintain open communication with care coordinators and guardians
· Complete monthly supervisions, noting a minimum of 1 hour per month
· Maintain Staff records-annual supervision plan, assurance of confidentiality, trainings
· Maintain individual’s records-annual consents, authorizations, ISP, Updates, Risk Assessment, SIS
· Maintain all records to ensure audit ready at all times
· Represent Alberta Professional Services in a professional manner at all times
· Maintain 24/7 on call status unless otherwise scheduled off
Minimum Qualifications:
1. An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human services profession, except a registered nurse who is licensed to practice in the state of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/IDD/SA with the population served; or
2. A graduate of a college or university with a Master’s degree om a human service field and has one year of full-time post graduate degree accumulated MH/IDD/SA experience with the population served, or a substance abuse professional who has one year full-time, post graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
3. A graduate of a college or university with a bachelor’s degree in human service field and has two years of full-time, post -bachelor’s degree accumulated MH/IDD/SA experience with the population served, or a substance professional who has tow years of full-time, post-bachelor’s accumulated supervised experience in alcoholism and drug abuse counseling; or
4. A graduate of a college or university with a bachelor’s degree in a field other than human service and has four years of full-time, post bachelor’s degree in accumulated MH/IDD/SA experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
