Purpose:
The overall goal of this position is to assist in ensuring compliance with agency policy and protocol as well as federal and state regulations. The position will be the interface between referral sources, the client and the agency personnel, prior to admission of client to home health care service. This will ensure client’s needs and expectations are properly managed.
Scope & Objective:
Education: Preferably one (1) year of experience as a Licensed Practical Nurse
Experience: Preferred one year of home care experience and intermediate level of experience with Excel
