Home Care Intake Specialist and Caregiver Program Coordinator
Bristol Elder Services, Inc., (Bristol) located in Fall River, MA, is a private, non-profit agency that provides community-based support services. We offer a comprehensive benefits package and competitive pay.
Bristol is hiring for a full-time position which will be a dual role consisting of an Intake Specialist and a Caregiver Program Coordinator. 35-hour work week. 9:00 a.m. – 5:00 p.m., Monday – Friday.
This individual will be responsible for completing initial in-home consumer assessments, service planning and coordination of care in compliance with Elder Affairs and agency guidelines within the 15 communities serviced by Bristol. This individual will also screen for public benefits and assist with applications for public benefits such as Mass Health, SNAP (Supplemental Nutrition Assistance Program) and fuel assistance.
As part of this position, this individual will also be promoting the agency’s Caregiver Support Program and will work closely with Bristol’s Caregiver Specialist delivering caregiver and professional education programs and coordinating community outreach efforts to promote the program. This individual will also provide back-up to the Caregiver Specialist in conducting initial assessments to caregivers and providing them with the information and resources to meet their needs as well as assisting in the coordination of special projects or grants as issued by Elder Affairs.
Bachelor’s degree in human services or a related discipline is required. Professional experience working with seniors or caregivers is beneficial. Candidates must possess excellent oral and written communications skills, ability to effectively relate to a wide variety of personalities, strong assessment skills, computer proficiency, organizational and time management skills, ability to multi-task, be adaptable and work in a fast-paced, team-oriented work environment. Driver’s license and reliable transportation required.
