Intake Services Representative - Home Care coordinates and facilitates the intake process for home care patients. Provides information about home care services, eligibility criteria, and policies to patients and families. Being an Intake Services Representative - Home Care utilizes health record management and other software to process the flow of documentation required to schedule and deliver services to patients. Responds to questions and resolves issues to ensure customer satisfaction. Additionally, Intake Services Representative - Home Care requires a high school diploma or equivalent. Typically reports to a manager or head of a unit/department. The Intake Services Representative - Home Care works under moderate supervision. · Positively engage clients in living purposeful and productive lives through whole life development and meaningful relationships and a gained sense of community contribution. Knows clients, ISPs, interests, issues and dreams and ensures that schedules and documentation reflect clients’ needs. Upholds standard DODD procedure, Downsize Farm philosophy, program quality, and client development.
· Develops meaningful relationships with clients and reports praises and/or concerns to residential manager. Takes pride in performance and compliance responsibilities, including med administration, personal funds, inventory, daily tasks, meal prep, incident reporting, emergency drills, and environment maintenance.
a. When applicable, ensures Navigator is given up to date documentation from all appointments and Maintains MARs to ensure that they are accurate and complete.
b. When applicable, Maintains Petty Cash logs and notifies manager if there is a discrepancy or if money is running low.
c. When applicable, Transport client for morning, evening, or daily scheduled activities.
· Maintains timeliness and dedication to the team in attendance, documentation, requesting off, and workplace positivity. If shifts cannot be covered – be solution oriented. Give as much notice as possible, find your own coverage when possible, and be creative in limiting workplace chaos.

· Additional Responisibilities
· Must attend Mandatory Quarterly Training Classes- provided by Downsize Farm
· Must maintain updated First Aid/CPR training and provide HR a copy
· Must maintain Med Admin Certification, if applicable and provide HR a copy
· Provide HR Manager with copy of updated Drivers ID, as applicable
· Provide HR Manager with copy of updated Private Auto Insurance, as applicable
· Must uphold all aspects of the Employee Code of Conduct
Qualifications for The Job
o High School diploma
o Meets eligibility and certification standards of DODD for Adult Services Employment.
o Valid drivers’ license with 6 points or less on driving record.
