We are currently looking for a Nurse Liaison Manager to join the team at our office in West Orange, NJ . The position would be responsible for assessing patients for home health care needs and coordinating referrals for home care and hospice services and ensuring the coordination and continuity of health care needs as patients transfer between different levels of care as well as providing supervision of the HCIC staff.
Responsibilities:
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers

Assesses client's total care needs

Provides individualized counseling to clients and their families to promote continuity of care

Coordinates referrals to home care from hospital and sub acute rehabilitation center

Provides direction and management of the homecare intake staff with the coordination of homecare referrals to promote continuity of care as well as acts as a resource for staff as needed

Establishes and maintains professional working relationship with all health care related staff and payers and VNAHG staff

Participates in patient-related Quality Improvement activities

Ensures compliance with regulatory and statutory requirements

Adheres to the organization’s policy in regards to absenteeism and appearance

Supervises the HCIC staff in conjunction with the organization’s managerial and leadership practices and manages the monthly schedule and time off request while operationally maintaining staffing

Qualifications:
- Bachelor’s degree in Nursing with valid NJ nursing license

2 years of community health nursing experience with prior management experience

Excellent verbal and written communication skills

Excellent interpersonal and organizational skills

Strong client focus, assertive, decision making skills, articulate, and creative in problem solving

Strong decision making skills
Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
