Job Description Summary:
Responsible for coordinating home care services for patients, which includes establishing and maintaining the patient’s plan of care, establishing and maintaining positive relationships with patients and related referral sources.
Essential Job Functions/Responsibilities:
1. Coordinates home care services for patients, assists with the establishment of the plan of care in coordination with the physician and staff, and ensures a seamless transition to home care.
2. Coordinates the implementation of all home care services, which includes nursing, physical therapy, occupational therapy, speech therapy, and personal care services, to ensure that quality service is provided to meet client needs.
3. Collaborates with inpatient utilization review departments and managed care Case Managers to facilitate safe, early inpatient discharge.
4. Works with Clinical Supervisors/Nursing Supervisors and staff clinicians to gather, collate, and report patient care summary information.
5. Actively participates in Interdisciplinary Team Meetings.
6. Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
7. Assists in building community, customer, payer and patient perceptions of Care Plus Nursing Services, Inc. as a high quality provider of services.
8. Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Care Plus Nursing Services, Inc..

Position Qualifications
1. Licensed nurse with current licensure to practice professional nursing in the state.
2. At least one year experience in care coordination preferred but not required.
