Position Summary:

Responsibility for population health program support outcomes including: production of consistent and timely reporting that measures progress of projects to population health stakeholders, integrator characteristics that include strong written, oral and electronic communication and knowledge to provide rapid and efficient patient access to population health programs and services.

Minimum Qualifications:

• Associates degree
• 3 years experience in outpatient program support or direct patient care role in a physician office practice, disease management program, homecare or other healthcare setting
• Technology expertise required (Google, MicroSoft including Powerpoint,Excel, Access Database and Social Media platforms).
• Able to work independently, be organized, mature, dependable and flexible.

Preferred Qualifications:

• Bachelor's Degree in a health related field
• 5 years experience in outpatient program support or direct patient care role in a physician office practice, disease management program, homecare or other healthcare setting
