We are looking for a full time Director of Case Management

Our Director of Case Management will lead our Case Management team. They are responsible for continuous improvement, and overall maintenance of the Utilization and Case Management Services. Responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a discharge plan in collaboration with the multidisciplinary team.

• Responsible for utilization review, discharge planning and care coordination with the patient, patient’s family, physician and health care team.
• Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
• Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
• Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition.
• Documents professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.

Qualifications

• Associate Degree in Nursing or Bachelor’s Degree in Nursing, BSN-Preferred
• Currently licensed as a Registered Nurse in the state of practice
• 2 years experience in nurse/clinical management
• 3 years experience in case management OR 3 years experience in clinical nursing
• Certification in Case Management, Preferred
• Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) will be considered for this position
