    POSITION DESCRIPTION:

    The Social Worker functions as a part of the multidisciplinary care team to plan, organize, and implement social work services for patients and families based on individualized needs. The Social Worker performs these functions consistent with the Utilization Review and Quality Improvement Plans with a patient-focused function to promote the restoration of health and the achievement of patient health goals.

    BRIEF DESCRIPTION OF DUTIES:

    • Perform psychosocial assessment of Social Work High-Risk Criteria cases.
    • Assess and assist patients and families to identify discharge barriers.
    • Work with an interdisciplinary team to develop a discharge plan for complicated psychosocial issues that are identified.
    • Provide information and referrals based on community resources and within patient insurance coverage.
    • Serve as a liaison between community agencies and hospital.
    • Supervision of profession social work students when appropriate.
    • All other duties assigned.

    SPECIAL SKILLS/EDUCATION REQUIRED:

    • NY State Licensed in Social Work required LMSW or LCSW.
    • Master's degree in social work.
    • Experience in patient discharge planning.
    • Knowledge of legal aspects of healthcare.
    • Excellent communication skills.

    • Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Southampton's Employee Health Services*
    • Successfully complete a 4-panel drug screen*
    • Meet Regulatory Requirements for pre-employment screenings.
    • Provide a copy of any required New York State license(s)/certificate(s).

    Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

    • The Human Resources department will be responsible for any fee incurred for examination.
