Resident Care Coordinator jobs in Logan, UT

Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor.     Description

    1. Adheres to and conveys a philosophy that supports the dignity, privacy, independence, choice and individuality of the residents.
    2. Perform personal care based on residents individual careplans.
    3. Communicate with the residents in a polite manner maintaining the residents dignity and demonstrating skills in communication with the residents
    4. Assist residents with medications and/or treatments as indicated in the Medication Record as assigned and allowed by regulations.
    5. Review Recent Notes in ECP and Sling Newsfeed (Apple Village/Apple Tree) on a daily basis for information about changes in resident needs and/or health conditions.
    6. Responds to residents calls on Notify and to other staff members on walkies in a timely manner.
    7. Demonstrates knowledge of and follows infection control procedures.
    8. Monitors for changes in residents needs, preferences, health status and reports/documents any changes according to established procedures.
    9. Responds to emergencies in an appropriate manner as per training received.
    10. Works effectively with minimal supervision and in a team setting.
    11. Performs other tasks and duties as assigned.
    12. All communication with families, physicians, nurses and pharmacies are in a caring and professional manner and are reported to the RN.
    13. Informs the Executive Director or Assistant Administrator of any problems concerning the building, housekeeping, kitchen, maintenance or financial issues

    Qualifications


    1. Be at least 18 years of age or 16 years if certified as a Nursing Assistant
    2. Must complete state approved Core Competency Training within 90 days of hire
    3. Pass a pre-employment drug screen
    4. Agree to have a criminal background check (requiring fingerprinting) through the Utah State Health Department
    5. Must have CPR and First Aide training within 3 months of hire date and kept current
    6. TB test taken within 14 days of hire
    7. Obtain a Davis County Food Handlers Permit within 14 days of hire
    8. Follow facility policy on grooming and dress code
    9. Have knowledge of facility policies and procedures
    10. Be trained and demonstrate competence to an RN in safely assisting residents in medication administration
    11. Be trained and demonstrate competency in observing and documenting changes in the residents physical and mental condition
    12. Understand resident rights
    13. Report any evidence of abuse or exploitation of the residents to the Executive Director
    14. All patient records and information are to be kept confidential. Inquires will be directed to the Executive Director
