Responsibilities:
- Provide care management services to individuals and families in need
- Conduct assessments to determine the needs and goals of clients
- Develop and implement care plans based on client needs and available resources
- Coordinate and monitor services provided by other professionals and agencies
- Advocate for clients and ensure their rights are protected
- Provide support and guidance to clients in navigating healthcare, social services, and other systems
- Utilize motivational interviewing techniques to empower clients to make positive changes in their lives
- Supervise and mentor junior care management staff

Requirements:
- Bachelor's degree in a related field (e.g., social work, psychology, nursing)
- Experience in care management, child welfare, or related fields preferred
- Knowledge of Medicare, Section 8, childcare, workforce development, or relevant programs
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
