    TITLE: Prevention Specialist Program Manager

    LOCATION: Tribal Health Department - St. Ignatius

    SALARY: $21.71 - $24.95 per hour plus benefits, may be employed under contract

    CLOSING DATE: Thursday, August 31, 2023 at 5:30 p.m. (MST)

    SPECIAL CONDITIONS:

    Perform work tasks as the Prevention Specialist in assigned reservation community/communities.

    Work tasks include prevention education, media campaign development, technical assistance,

    program implementation, and other tasks as required through specific grants and/or Tribal Health

    priorities. Responsible to report grant-specific data and/or other required data per individual grant

    specifications and Tribal Health priorities.

    Prevention Specialist will provide prevention and education services at a community level through

    evidence-based practices according to grant and/or Tribal Health priorities. The Prevention

    Specialist will work with and provide support to prevention staff in developing, implementing, and

    monitoring projects and will engage community members and partners in carrying out the tasks

    assigned as outlined in the specified grant and/or Tribal Health priorities.

    Strategic Planning – Identify and evaluate areas of need and opportunities for improvement by

    analyzing data and creating a Logic Model and Action Plan. Implement evidence-based programs

    in the community based on need. Provide support to community partners in fulfilling

    organizational objectives. Partnership Development – Develop strategic alliances with

    representatives from all community sectors including but not limited to parents, families, schools,

    health care, public health, and youth-serving organizations. Work closely with community

    coalitions to carry out goals of the Logic Model. Program Delivery – Ensure the effective

    operation and delivery of programs within the service area. Ensure the integrity of

    science/evidence-based programs are maintained and delivered in the manner identified within the

    timeline established.

    1. Interpret and apply the steps of prevention framework to best serve the identified needs

    of the community, including developing a logic model and action plan.

    2. Develop and deliver educational presentations throughout the assigned service area to

    target audience(s).

    3. Provide training and presentations to community groups, agencies and youth on

    prevention topics identified in the logic model.

    4. Provide technical assistance and participate in local coalitions with common goals.

    5. Assist with development of a regional media plan, as applicable, by providing technical

    assistance and implementation in coordination with other local prevention

    coalitions/organizations.

    6. Consult, support and work with local prevention staff in planning and implementation

    of prevention strategies.

    7. Record and report data as required by the grant/program.

    8. Develop and update prevention plans as required for grant/program, as applicable.

    9. Maintain all program records in a professional and timely manner and in accordance

    with programmatic and grant/program requirements.

    10. Attend and participate in all required meetings as required.

    11. Attend and participate in all contract-required prevention training.

    12. Review available data to support projects identified in logic model.

    13. Coordinate with schools and/or youth groups to implement evidence based programs.

    14. Conduct presentations, meetings, focus groups and key informant interviews with

    community stakeholders to inform program objectives

    MINIMUM QUALIFICATIONS (AS REFLECTED ON THE TRIBAL EMPLOYMENT

    APPLICATION):

    Bachelor’s degree in a health related, social work, or human services field. Minimum 1 year

    working in public/community health and/or prevention. Experience managing budgets and writing

    reports. Experience managing grants, preferred OR an Associate degree in a health related, social

    work or human services field. Minimum 3 years working in public/community health and/or

    prevention. Experience managing budgets and writing reports. Experience managing grants,

    preferred OR Minimum 5 years working in public/community health and/or prevention.

    possess a valid driver’s license.
