Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Responsibilities of a Registered Nurse (RN):
• Follow a designated care plan in accordance with patient's needs• Make home visits to clients in designated geographic territories• Perform assigned duties, including administration of medication, wound care, treatments, and procedures• Monitor clients' conditions; reporting changes to Leadership• Follow up with, execute, and properly document doctors' orders• Perform client assessments as necessary• Case management and coordination• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tabletQualifications for a Registered Nurse (RN):
• Be a graduate of an accredited School of Nursing.• Current state license and CPR certification.• Possess one (1) to two (2) years of recent acute care experience in an institutional setting is preferred.• Possess one (1) to two (2) years of recent experience in Home Care or Home Health is preferred.Our employees are our greatest asset:
