  • Support Incorporated is seeking to hire an additional Care Manager to join our team. This position offers flexible hours and will involve a combination of on-site and remote work arrangements.

    Service Duties and Responsibilities:

    • Complete the Comprehensive Care Management Assessment annually at enrollment or when significant changes occur.
    • Develop Care Plans derived from the completed assessments.
    • Coordinate and facilitate Care Team Meetings to ensure successful implementation of the plan.
    • Collaborate actively with the care team, supported members, and service providers to ensure the development of a plan that accurately reflects the individual’s needs and desired life goals.
    • Coordinate with other team members to ensure smooth transition to appropriate level of care.
    • Update other Care Team members of urgent or pertinent treatment updates
    • Schedule face-to-face meetings with members/LRP to provide education about care teams, services, needed supports, etc.
    • Provide comprehensive education and support to individuals and LRP regarding their rights, grievance and appeals process, available service options, providers capable of meeting their needs, and payer requirements that may influence service connection and maintenance.
    • Escalate complex cases and cases of concern to the supervisor.
    • Promote customer satisfaction through ongoing communication and timely follow-up on any concerns/issues.
    • Document all applicable member updates and activities per organizational procedure.
    • Ensure that service orders/doctor’s orders are obtained, as applicable.
    • Obtain releases/documentation and provide them to all stakeholders involved.
    • Obtain clinical supporting documentation, legal/guardianship verification, and necessary consents to exchange/release information
    • Demonstrate a comprehensive understanding and adherence to all agency policies and procedures and state and federal statutes and regulations pertaining to care management.
    • Meet at least minimum standards of monthly contacts and demonstrate ability to effectively engage with members.
    • Care Manager will coordinate the member’s health care and social services including behavioral health, LTSS and pharmacy services as well as other SDOH needs.
    • Provide comprehensive education to members on self-management of all aspects of their care, encompassing medication, services, and natural and community resources available to them.
    • Provide education and guidance on self-advocacy to the member, family members, and support members.
    • Connect members and caregivers to education and training to help the member improve function, develop socialization and adaptive skills, and navigate the service system.
    • Provide information and connections to needed services and supports including but not limited to self-help services, peer support services, and respite services utilizing partnerships with providers in the community as well as utilization of NCCARE 360 to make referrals, as needed.
    • Promote wellness and prevention programs, including ensuring annual wellness visits.
    • Provide information on establishing advance directives, including psychiatric advance directives as appropriate, and guardianship options/alternatives, as appropriate; Connecting members and family members to resources that support maintaining employment, community integration, and success in school, as appropriate.
    • Address, in the Care Plan, if member has identified Social Determinants of Health (SDOH), disparities and/or complex payer issues.
    • Assist individuals/legally responsible persons (LRP in choosing service providers, ensuring objectivity in the process.
    • Utilize ADT feed information to respond within hours/minutes as appropriate to support members who are admitted, transferred or discharged from a facility in a timely manner.
    • Consistently evaluate the appropriateness of services and ensure the implementation of the plan of care through information gathering and assessment at a defined frequency of contact based on risk stratification.
    • Utilize person centered planning, motivational interviewing and historical review of assessments to gather information and to identify supports needed for the individual.

    Support, Inc. is a CABHA agency that has been CARF accredited since 2008 providing a continuum of Mental Health and Substance Use services for over 20 years. Support, Inc. provides the full array of basic and enhanced benefit services, with the exception of Level III and PRTF services.
